Healing Items
Food and drink never heal you. They only fill hunger and thirst. But when either gets low, your health bar gets a dark tail: health you can't get back until you eat or drink. At zero, you're capped at 40%. On top of that, natural regeneration stops at about 62%. Anything above that takes a healing item or a medic.

Pills are a stopgap, not a cure. Keep redosing painkillers or Zombix while you're badly hurt and your character builds up lasting injury. First every heal gets weaker. Then you can't sprint properly (“You can barely keep going without a hospital.”). Stop redosing and it fades over time. Get real treatment from EMS or a hospital bed.
Bleeding & broken bones
- Big hits make you bleed. Bleeding gets worse over time and moving makes it faster. Eventually your screen blacks out.
- Leg injuries slow you down. Arm and head injuries have their own effects.
- Bandages and medical kits treat these. Painkillers hide the symptoms for a while.
Going Down & Hospital
- When you go down, you have about 5 minutes for someone to reach you: EMS or another player.
- After that you respawn at the hospital. You keep your inventory.
- Hospital check-in at Pillbox costs $2,000. You lie in a bed for about 20 seconds and walk out healed.
- Wear your seatbelt (K). Without one, a hard crash throws you through the windscreen.
